Required Enrollment Documents

Please be advised that your child is not guaranteed a spot in our center until 

ALL of the required documents are received and you have been approved for enrollment by the Administrator

Medical Statement

This form must be completed for every child who is not enrolled in school. It must be completed by a  licensed physician or nurse practitioner. Don't forget to attach your child's shot record.

Child Enrollment Form

An Enrollment form must be completed for each child you wish to enroll. Please complete all fields. You must list the name and number of a physician and at least 2 emergency contacts. Don't forget to check the box indicating you have reviewed the Parent Handbook. Click on the icon above to print the Child Enrollment Forms

Family Information Sheet

This form is used exclusively by Smart Start to get additional information about your child and family to help familiarize our staff with your child to help ease the transition into our program. Click on the icon above to print the Child Information Form
